- How Do I Get The Rebate Form? To receive a Rebate Form, simply check the box on the product page that says, "Send rebate form.". Rebate instructions will be provided with your order. If you do not receive the rebate slip with your order, it can be printed from www.acuvue.com/rebates
- Complete the Rebate certificate provided
with your order or printed from the acuvue.com website and attach the appropriate number of box tops as specified on the rebate form.
- Attach receipt(s) for the eye exam AND product
purchase. Purchase must be made withing 90 days of eye exam.
